Thinking Ahead…
If you think your scout will be attending Treasure Valley this year, you’ll need to have your
pediatrician complete part B of the enclosed medical form.
Please bring part B of the medical form (link below) to your sons’ next physical and have the doctor
complete and sign part B (and attach a copy of his immunization record).
All other documents will be sent at a later date, this just gives you the opportunity to have the forms
completed during a routine scheduled appointment rather than chasing your doctor last minute in
July. Please note that since the form must be updated annually, a copy of last years’ form will not
be accepted.
